package must accompany form. Limit one towel per customer/household.
Requests must be postmarked no later than August 1, 2008. Gill North
America reserves the right to substitute product of equal value.
Requests from groups, post office boxes or organizations will not be
honored. Purchases by re-sellers and dealer employees are ineligible.
